Who Is Eligible for the CDCP

The Canadian Dental Care Plan is available to eligible Canadians who meet all of the following criteria:

• No existing private dental insurance
• Adjusted family net income under $90,000
• Resident of Canada for tax purposes
• Filed an income tax return for the previous year

How to Apply for the Canadian Dental Care Plan

To apply for the CDCP, you may need the following information:

• Social Insurance Number, if available
• Proof of income
• Full legal name and date of birth
• Home and mailing addresses
• Details of any other government benefit programs
• Most recent tax return and Notice of Assessment
